Evolvement of a warfarin medication therapy adherence clinic in Malaysia and its impact on international normalised ratio control over time

Warfarin is a drug of narrow therapeutic index, and the use of this agent for anticoagulation requires close monitoring to ensure efficacious treatment and to prevent serious adverse effects such as haemorrhagic complications. Evidence suggests that pharmacist-led anticoagulation services may be an effective means to manage warfarin anticoagulation. This report describes experience of the Malaysian context regarding the operations of a warfarin medication therapy adherence clinic.
